Case Study: Pulmonary Hypertension Revealed by EchoRocky, a 14-year-old Pomeranian, presented to his primary care team for panting, lethargy, and increased respiratory effort. He became cyanotic and collapsed on the way to the hospital. Chest radiographs revealed mild pleural effusion and a pattern consistent with bacterial or fungal pneumonia, but with a normal cardiac silhouette. A same-day echocardiogram by Mobile Veterinary Imaging helped clarify the picture. Our scan revealed severe pulmonary hypertension with marked right ventricular hypertrophy, right atrial enlargement, and significant tricuspid regurgitation. Left-sided heart function was normal, and no pericardial effusion was present at the time of the study. These findings shifted the clinical focus. Rather than starting Rocky on diuretics, his primary care team prioritized pulmonary support and started targeted therapy for pulmonary hypertension. Immediately following Rocky's scan, we submitted his images to our partner cardiologist for a STAT consult. Our collaborative efforts allowed for rapid interpretation and specific treatment recommendations—starting him on sildenafil and pimobendan while he remained stable. Quick coordination between imaging and cardiology allowed for same-day treatment decisions and supported ongoing in-hospital care.
